*THE BLUFF CITY VOLUNTEER FIRE DEPARTMENT REQUIRES ALL MEMBERS TO HAVE A YEARLY PHYSICAL. THE DEPARTMENT PAYS FOR THE PHYSICAL AND IT IS CONDUCTED AT PINEY FLATS URGENT CARE. ARE YOU WILLING TO SUBMIT TO A YEARLY PHYSICAL?
Yes  NO  *THE BLUFF CITY VOLUNTEER FIRE DEPARTMENT RECOMENDS HEPATITIS A AND B SHOTS. THIS IS NOT REQUIRED. THE DEPARTMENT PAYS FOR THE SHOTS AND THEY CAN BE OBTAINED THROUGH THE SULLIVAN COUNTY HEALTH DEPARTMENT. ARE YOU WILLING TO HAVE THE HEPATITIS A AND
